Job Purpose

The role of the Clinical Nurse Manager II Discharge Coordinator is to provide leadership and co-ordination to the wards and specialty teams with regard to patient discharge across Connolly Hospital. To play a key role in the facilitation of timely and seamless discharge of patients. The successful candidate will be responsible for monitoring and reporting of complex discharges and escalating any issues as appropriate.

 

The salary scale for the post is: (01/02/2026)

€62,078 €63,106 €63,975 €65,394 €66,963 €68,504 €70,045 €71,779 €73,389 €76,159 €78,443 LSI

 

There is currently 1 permanent whole-time vacancy available in the Patient Flow Operational Hub.

 

A panel may be formed as a result of this campaign for CNM II Discharge Coordinator from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

Essential Criteria

  1. Each candidate must, at the latest date for receipt of completed applications for the post possess:

     

    1a. Professional Qualifications & Experience

     

    • Are registered in the relevant Division of the Register of Nurses kept by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered

     

                                                                        And

     

    • Have at least 5 years post registration experience (or an aggregate of 5 years post registration full time experience) of which 2 post registration full time experience (or an aggregate of 2 years post registration full time experience) must be in the specialty area or related area

                                                                        And

     

    • Have the clinical, managerial and administrative capacity to properly discharge the functions of the role

                                                                        And

     

    • Candidates must demonstrate evidence of Continuing Professional Development

     

                                                                       And

     

    (b) Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.

  2. Annual Registration

     

    (i) On appointment, Practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Board Altranais agus Cnáimhseachais na hÉireann).

                                                                           And

     

    (ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

  3.  

     Health

    A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

  4.  

     Character

    Each candidate for and any person holding the office must be of good character.
