The successful candidate will work as a main grade Social Worker on an acute medical ward in Connolly Hospital. MDT co-working and leading out on case management and coordination is essential to working in this setting. The key role and responsibilities are providing patient centred high quality care, supporting patients and their loved ones navigating the health system. Key to this is supporting them with any identified psychological, emotional, social or practical barriers to discharge. Coordinating care, signposting to relevant community based services and multi-agency collaboration is key to the execution of this role.
Contribution to quality improvement and practice development is a key part of this role. The successful candidate must demonstrate strength in managing a fluctuating caseload whilst providing quality of care to those they support, optimising their physical, mental and emotional health during their hospital stay and their transition back into the community setting.

Statutory Registration, Professional Qualifications, Experience, etc
(a) Candidates for appointment must:
(i) Be registered, or be eligible for registration, on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
AND
(ii) Have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office.
AND
(iii) Provide proof of Statutory Registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU before a contract of employment can be issued.
Annual registration
(i) On appointment practitioners must maintain annual registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
AND
(ii) Practitioners must confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).
Health
Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
